Serotonin syndrome

Recent start or dose increase of a serotonergic agent (SSRI, SNRI, MAOI), often in combination.

How it presents

autonomic instability, hyperthermia, clonus, hyperreflexia, agitation, tremor, diarrhea

Tell it apart from

  • Neuroleptic malignant syndrome
  • Malignant hyperthermia

Treatment reasoning

Stop the serotonergic agent; supportive care, cooling, benzodiazepines for agitation; cyproheptadine in severe cases.
