Neuroleptic malignant syndrome

After recent antipsychotic (esp. first-generation high-potency) initiation or dose increase.

How it presents

fevers, severe muscle rigidity, altered mental status, autonomic instability, elevated CK

Tell it apart from

  • Serotonin syndrome
  • Malignant hyperthermia
  • Catatonia

Treatment reasoning

Stop the antipsychotic; supportive care, cooling, hydration; dantrolene and/or bromocriptine in severe cases.
