Bipolar I disorder

At least one manic episode; may have had depressive episodes.

How it presents

manic episode: elevated/irritable mood, grandiosity, decreased need for sleep, pressured speech, flight of ideas, increased goal-directed activity, reckless behavior; lasting ≥1 week

  • Bipolar II disorder
  • Cyclothymic disorder
  • Schizoaffective disorder
  • Stimulant intoxication

Treatment reasoning

Mood stabilizers (lithium first-line for mania), atypical antipsychotics; adjunctive psychotherapy; discontinue antidepressants in acute mania.
